You can add your favorites to your <a href=\"\/fr\/how-it-works\/favorites\/\" rel=\"noopener\" target=\"_blank\">Favoris<\/a>. Once you&#8217;ve decided on your favorite donors, you can explore\u252c\u00e1more in-depth\u252c\u00e1information such as: written staff impressions, childhood photos,\u252c\u00e14 generation family medical history, personality exams and audio interviews so you can hear their voices and get a feel for\u252c\u00e1their who they are.<\/p>\n<p><strong>If your chosen donor(s) shows no available vials, please call us\u252c\u00e1to verify\u252c\u00e1availability. We sometimes have units in stock that are not listed on the website.<\/strong><\/p>\n<h3>Step 2: Fill Out Related Paperwork<\/h3>\n<p>Before we send out your shipment, we ask that you fill out the\u252c\u00e1forms related to your purchase:<\/p>\n<ul>\n<li>Our <a href=\"\/fr\/forms\/\" target=\"_blank\">Purchaser Semen and Storage Agreement <\/a>outlines the terms you and Seattle Sperm Bank agree to in purchasing, storing and using donor sperm. Every client is required\u252c\u00e1to complete this form and return it to us before vials are shipped out.<\/li>\n<li>The <a href=\"\/fr\/forms\/\" target=\"_blank\">Clinical Release <\/a>form is required for orders\u252c\u00e1being picked up from our office or those\u252c\u00e1delivered to any location that is not a clinic (i.e. home delivery). It\u252c\u00e1provides written authorization from your\u252c\u00e1licensed health care professional that they are overseeing your care. This form is mandatory for all shipments being delivered within\u252c\u00e1New York State and\u252c\u00e1must\u252c\u00e1be signed by a physician, physician\u0393\u00c7\u00d6s assistant, or nurse practitioner.<\/li>\n<\/ul>\n<p>Both forms may be submitted electronically or via\u252c\u00e1fax or email.<\/p>\n<h3>Step 3: Place Your Order for Donor Sperm<\/h3>\n<p>Once you have decided on a donor, you can order the vials directly on our website or over the phone with one of our staff members.<\/p>\n<p>We accept all major credit cards, including Visa, MasterCard, Discover, and American Express. Please know the vial type and quantity you wish to order at\u252c\u00e1the time of purchase.<\/p>\n<h3>Step 4: Shipping or Storing Your Vials<\/h3>\n<p>When you place your order, you will decide whether to store the vials with us, or if you want the vials shipped to your home or clinic.<br \/>\n<strong><br \/>\nWe encourage patients to schedule the arrival of the units at least 2-3 days prior to the scheduled insemination date.<\/strong> Standard shipping within the United States is two business days. Overnight shipping is available to some domestic locations for an additional fee, but overnight shipping is not guaranteed by our carriers UPS and FedEx.<\/p>\n<p><strong>Ship early &#8211; Plan Ahead.
